Purpose of study= To determine the outcome of severe senile aponeurotic ptosis corrected by levator aponeurosis advancement.

Design of study= Hospital based prospective study.

Period of study = From October 2001 to September 2011

Materials and Methods: Twenty six cases with severe senile aponeurotic ptosis of >4 mm with fair levator function of 5-7 mm and typical features of levator disinsertions were included in this study. The cases with ptosis of cause other than old age were excluded. All cases were treated by levator aponeurosis advancement on anterior surface of tarsal plate under local anaesthesia and followed-up for 6 months to see post operative ptosis recurrence and complications. The recurrence was defined as ptosis of 2mm or more.

Results: We studied on 45 senile ptotic eye lids in 26 cases having age range from 58- 74 years (mean age 61.3 years). Among those, 16 (61.53%) were male, 1o (38.46%) female, with bilateral ptosis in 19(73.07) and unilateral in 7(26.92%) cases. The good outcome with satisfactory functional results was noted in 32(71.11%) lids (p=0.000), fair outcome with incomplete recurrence of 2 mm ptosis in 5 (11.11%) lids, Poor outcome with complete recurrence of ptosis >4mm in 2(4.44%) lids, lagophthalmos in 2(4.44%) lids and poor lid contour in 2(4.44%) lids.
